Dr.RAJYALAKSHMI DEVI TANUJA.P BEARING REGNo. 60316 DATED : 05-08-2008 IS REMOVED FROM MEDICAL REGISTER ON 21-01-2015 FOR A PERIOD OF SEX (6) MONTHS. APMC/DC/015/1301/Case No. 33 of 2013.

In pursuance of decision dt: 30-12-2014 taken by the General Body of APMC, the name of Dr. Rajyalakshmi Devi Tanuja P bearing Reg.No. 60316 dated: 05-08-2008 is removed from Medical Register on 21-01-2015 for a period of six (6) months as provided in Section 17 (2) R/W Section 15(4) of Andhra Pradesh Medical Practitioners Registration Act, 1968 as there was negligence on her part in assessing the required levels of Anesthesia resulting in conversion from spinal anesthesia to general anesthesia which could have been avoided. Hence there are lapses in taking adequate care of the patient, namely, Seela Tulasiram which has resulted in his death.

In view of the above order, as provided in Sub-section (5) of Section 17 of the said Act, Dr.Rajyalakshmi Devi is hereby directed to surrender her Permanent Registration Certificate bearing No. 60316 and the certificate of Registration of Additional Qualification forthwith.
